How to request certificates (for graduate)

Certificates can be requested at the Student Affairs office or by postal mail.

Office hour
Monday-Friday: 8:30-12:00, 13:00-17:00
Please note that requests for Certificate(s) can’t be processed on the following days.
  1. Saturday, Sunday and public holidays in Japan
  2. December 29th through January 3th on New Year’s Holidays
  3. August 13th through August 15th on University Designated Holiday

About protection of personal information
In accordance with the university's Policy on the Protection of Personal Information, we issue certificates based on the request by the person himself/herself. If a proxy person requests for you, then you must submit a letter of proxy (original) and the ID of the individual serving as your proxy person (photocopy).

NOTE
Requests by either phone, facsimile or e-mail are NOT accepted.
Certificate(s) will be issued by the full name at the time of your graduation/withdrawal from each University.

Certificates processing fee

All Certificates Processing Fee is 300 JPY (per 1 copy)

Day required

The day to issue/dispatch documents depend on the day the request was completed.

1. How to request within Japan

(1) How to request

(1)-a. At the office

(1)-a.-1.Please make sure to bring the following 2 items
I. ID: driver's license, passport, resident card, or jumin-kihon-daicho card
II. Cash to pay the processing fee

You must pay for the processing fee and postage.
All Certificates Processing Fee is 300 JPY (per 1 copy)

(1)-a.-2. Procedure

Fill in the certificates request form at the Student Affairs office.
* Please submit your above-mentioned ID.

(1)-b. By postal mail

Please send us the following 3 items by postal mail.
I. Request form (please send the original request form.)
Please use the PDF files above「Request for Certificate(s)」
II. Photocopy of your ID (driver's license, passport, resident card, or jumin-kihon-daicho card)
III. Fee

The payment is to be made by "Postal Money Order(Teigaku Kogawase)", which is available at post office.※All Certificates Processing Fee is 300 JPY (per 1 copy)
Please enclose "Postal Money Order (at value of the total fee)", leaving the recipient blank, with a request form.
In case you need the certificates to be sent by express mail, the charge of 280JPY should be paid by postage stamp.

(2) Receipt of certificates

(2)-a. Pick up at the office

If requested at the office ;
Please bring the receipt of the application form and the above-mentioned ID.

If requested by postal mail;
Please bring the above-mentioned ID.

* Please note that the certificates will be disposed if you do not pick up within 3 months of the date of issue.

(2)-b. Receive the certificates by postal mail

The certificates will be send to the name/address stated on the request form. Please refer to the above-mentioned “Day Required “ regarding to the day to dispatch the certificates.

2. How to request from overseas

(1) How to request

If you live in countries outside Japan, you can request Certificate by your proxy person who lives in Japan.

Please send your proxy the following 3 items by postal mail.
I. Request form (please send the original request form.)
Please use the PDF files above「Request for Certificate(s)」
II. Photocopy of your ID (driver's license, passport, resident card, or jumin-kihon-daicho card)
III. An original letter of Proxy (it must include the signature of the requester)
Please use the PDF file above「letter of proxy」

(2) Procedure by a proxy (at the office)

Please make sure to bring the following 5 items
1). Request form (the above-mentioned 2-(1)- I)
2). Photocopy of your ID (driver's license, passport, resident card, or jumin-kihon-daicho card) (the above-mentioned 2-(1)- II)
3). An original letter of Proxy (the above-mentioned 2-(1)- III)
4). Photocopy of your proxy person’s ID (driver's license, passport, resident card, or jumin-kihon-daicho card)
5). Fee
